The Right Services

An individualized plan of care is arranged by the nurse in collaboration with you, and your physician. Our services focus on meeting each patient's unique health care needs.
You have the right to:
1. be informed of your rights.
2. complete and competent care of the highest quality.
3. courteous, fair and respectful care and treatment of you and your property.
4. prompt response to requests for help and services.
5. service without regard to race, color, cered, sex, age, national origin, ethnic origin, disability or sponsorship.
6. know the title, name, and purpose, through picture identification cards, of all people entering your home.
7. be notified in advance of all changes in your plan of care.
8. participate in the development of your plan of care and your discharge from homecare.
9. refuse treatment and/or medication after being fully informed and understanding the consequences of each action.
10. privacy, including confidential treatment of your record except as required by law or third party payment contract.
11. submit complaints about the care and services provided without fear of discrimination, reprisal, coercion or eduction of services.
12. be informed of the reason for discharge from this program, of your continuing needs and alternative levels of care for meeting those needs.
13. register a compalint with the NYS Department of Health Hotline at 1-800-628-5972.
14. have these rights exercised by another individual, guardian or legally authorized entity if you are unable to do so.
